NEW PATIENTS

We are happy to accept new patients for comprehensive treatment, consultations or second opinions. We accept most dental insurance plans; if you have any questions about your coverage, please call our office, in most cases we can check if we can work with your insurance before you come in. We do not accept Denti-Cal. To save time, please download and print the Patient History and Information form HERE. Please be sure to circle either yes or no for each of the health history questions. If you are taking prescription medications, please list them in the appropriate box or on a separate sheet of paper. We look forward to meeting you!
